Bridging the Gap Between Higher Levels of Care and Outpatient Therapy
Leaving a psychiatric hospital, PHP, or IOP can be a significant transition. Even when someone is doing better, moving from an intensive treatment environment to traditional outpatient therapy can feel abrupt.
The Bridge Program was created to make that transition smoother.
The Bridge Program is an individualized transitional care program designed for clients stepping down from hospitalization, PHP, or IOP into ongoing outpatient psychotherapy.
Rather than waiting until discharge to establish a new therapeutic relationship, the Bridge Program allows the client’s outpatient therapist to begin connecting with them while they are still in their higher level of care.
A Warm Handoff, Not a Cold Start
One of the central components of the Bridge Program is the warm handoff.
While a client is in hospitalization, PHP, or IOP, their assigned Bridge Program therapist can begin meeting with them virtually or in person. When appropriate, the therapist can also communicate with the existing treatment team and participate in transitional and discharge planning.
This means that when the client leaves a higher level of care, they already know who their outpatient therapist is—and their therapist already understands the treatment journey that brought them there.
What’s Included
Individual Therapy
- Typically twice weekly during the first few weeks
- Frequency individualized based on clinical need and progress
- Gradual adjustment of services as the client stabilizes
Family Therapy
- Typically once weekly, particularly during the initial weeks
- Frequency individualized based on clinical need
- Can be provided by the individual therapist or another designated clinician
Monthly Group Therapy
- One group therapy day each month
- Provides additional therapeutic support, connection, and skill development
Family Coaching
- Available as an additional service when needed
- Provides families with practical support and guidance as they navigate the transition home and ongoing treatment
Transitional Care & Coordination
- Therapist connection can begin before discharge
- Virtual or in-person sessions available
- Collaboration with the hospital, PHP, or IOP treatment team when appropriate
- Focus on continuity, communication, and a coordinated transition to outpatient care
Individualized for Every Client
The Bridge Program is not a one-size-fits-all treatment model.
The program is completely individualized and the intensity and frequency of services are determined by each client’s clinical needs. As clients gain stability and confidence, treatment can gradually transition toward their ongoing outpatient therapy plan.
Who Is the Bridge Program For?
The program may be appropriate for individuals who are:
- Transitioning from psychiatric hospitalization to outpatient care
- Stepping down from PHP
- Stepping down from IOP
- Benefiting from additional structure during the transition to outpatient treatment
- In need of increased family involvement during a period of stabilization
- Ready to continue their progress in a less intensive setting but would benefit from a gradual transition
